Final fix for loading audio error and death scene

I finally squashed the last audio bug. Saving and loading now work, except if you exit the game, all equipment can still be lost. I will have to spend a little time learning more about how to save equipment. I also added a death scene. For now, it is very similar to the starting scene. I also changed the order of the intro and start scene, so that you can continue without watching the scene again. So far the battle mechanics are holding up, but if you lose equipment at the end of the game, it makes it very difficult to fight in the tomb.
